29 CFR 1926.960(c)(1)(iii)(B): The energized part was not insulated from the employee and from any other conductive object at a different potential:    On or about 12/7/2016, an employee received 2nd and 3rd degree burns from an arc flash while part of his upper right arm area made contact with an uninsulated area on "C" phase while his right hand was untying the insulator holding "B" phase resulting in a phase to ground arc flash due to a differences of potential.     PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.960(c)(2)(i)(B): When installing insulation for purposes of paragraph (c)(2)(i)(A) of this section, the employee installs the insulation from a position that does not expose his or her upper arm to contact with other energized parts:    On or about 12/7/2016, an employee installing line covers on conductors that were 13.2KV phase to ground and 23KV phase to phase was not wearing rubber sleeves when his upper right arm made contact with an uninsulated area on "C" phase while untying "B" phase from the insulator resulting in an arc flash.    PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

The employer did not ensure that each employee, to the extent that other safety-related conditions at the worksite permit, works in a position from which a slip or shock will not bring the employee's body into contact with exposed, uninsulated parts energized at a potential different from the employee's:    On or about 12/7/2016, the employer did not ensure an employee worked in a position that would prevent him from his body making contact with uninsulated part on "C" phase, which had a break between the line hose that eventually caused an arc flash due to a differences of potential.    PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.960(g)(2): For each employee exposed to hazards from electric arcs, the employer did not make a reasonable estimate of the incident heat energy to which the employee would be exposed:    On or about 12/7/2016, a foreman and qualified observer did not make a reasonable estimate of the incident heat energy when a lineman working on 13.2KV to 23 KV powerlines without rubber sleeves or face protection when the "C" phase was not fully covered with line hoses exposing a small break between the insulation and ultimately causing a phase to ground arc flash.     PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
