29 CFR 1910.23(a)(6): Manhole floor opening(s) with the cover(s) removed were not constantly attended by someone or protected on all exposed sides by removable standard railings:  a) Sewage Pump Station at the Orange County Correctional Facility - On or about December 2, 2016, an employee was pumping sewage out of a sewer pit with the hinged cover of the manhole opened exposing the employee to fall hazards of approximately 24 feet.  The employer did not protect the open manhole floor opening with removable standard railings and/or did not have someone constantly attending the manhole.
