29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   (a) On or about November 9, 2016, and at certain times before, employees were exposed to potentially serious injuries and/or illnesses in that the employer had not developed, or successfully implemented, an effective safety and health program.  On November 9, 2016, at a job site located at 625 S. 4th Street in Columbia, Missouri, employees that were working from a scaffold were exposed to fall hazards due to their not having been trained in the regulations applicable to their work.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   (a) On or about November 9, 2016, at a job site located at 625 S. 4th Street in Columbia, Missouri, employees were exposed to fall hazards while working from multiple scaffold platforms where the platforms were not fully decked between the front uprights and the guardrail supports.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph:   (a)  On or about November 9, 2016, at a job site located at 625 S. 4th Street in Columbia, Missouri, employees were exposed to scaffold collapse and fall hazards while working from a scaffold with a height to base width ratio greater than 4:1 that was not tied back to the building or otherwise restrained from tipping.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundations:   (a) On or about November 9, 2016, at a job site located at 625 S. 4th Street in Columbia, Missouri, employees were exposed to fall and scaffold collapse hazards due to some scaffold frames sitting on unstable footings in that several instances were noted where the frames and base plates were sitting on top of paving blocks stacked 2 feet high.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:   (a) On or about November 9, 2016, at a job site located at 625 S. 4th Street in Columbia, Missouri, employees were exposed to strains, sprains and fall hazards while accessing working platforms by climbing the tubular, welded-frame scaffold sections.  The scaffold sections were of the masonry walk-through type that do not provide integral scaffold access.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above a lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   (a) On or about November 9, 2016, at a job site located at 625 S. 4th Street in Columbia, Missouri, employees were exposed to serious fall hazards while working from scaffold platforms that lacked guardrail systems.  One employee was observed to be wearing a personal fall arrest system, but his lanyard was "anchored" to a scaffold cross member which does not meet the requirement for a fall protection anchorage point.
