29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentally walk shall be guarded by either:  A floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing.  a) On November 21, 2016 while employees were cleaning debris from the roof, guards were not in place to prevent from falls. The employee stepped upon the roof upper section felling through the plywood covering the opening, exposing employees to a fall height of 10 1/2 foot to the floor below.
