29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:    a)  Ohio and Indiana Roofing on or about December 05, 2016, at the address of 249 N. Main St. Delphos, OH: The employer did not ensure that the employer fabricated horizontal lifeline was designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system. Employees utilizing the horizontal lifeline during low-sloped roofing operations were exposed to potential falls in that the employer's design and installation of the system did not incorporate engineering data, identifying the specifications of installation and limitations of use. Employees were unable to effectively assess the parameters in which the protective system may be safe to utilize.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a)  Ohio and Indiana Roofing on or about December 05, 2016, at the address of 249 N. Main St. Delphos, OH: The employer did not ensure an employee was protected from falls while performing roofing activities atop a low sloped commercial structure. The employee performing clean-up activities at or near the roof edge (outside warning lines) without the benefit of fall protection was exposed to an approximate 20 ft. fall to the level below.      Ohio and Indiana Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1187175, citation number 1, item number 1 and was affirmed as a final order on November 28, 2016, with respect to a workplace located at 115 E. Broad St., Columbus, OH.
