29 CFR 1910.37(a)(2): Exit routes were not arranged so that employees did not have to travel toward a high hazard area, unless the path of travel was effectively shielded from the high hazard area by suitable partitions or other physical barriers:  a) On or about January 27, 2017 in the entrance/ exit door to 2080 Dr. Andrew Brown Avenue, Indianapolis, Indiana, an open flame propane heater was in the access area to the exit and approximately 240 pounds of propane was stored next to the exit door.

29 CFR 1910.37(b)(1): Each exit route was not adequately lighted so that an employee with normal vision can see along the exit route:  a) On or about January 27, 2017 in the entrance/ exit door to 2080 Dr. Andrew Brown Avenue, Indianapolis, Indiana, no emergency lighting or other illumination was provided in the exit route or at the exits. The decontamination room and the clean room through which employees were to exit contained no windows or other source of illumination.
