29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about at 12/8/16 located at Southwick Dr., Cumming, GA: Employees were installing tar paper and roofing shingles on to the 8/12 pitch roof on the house that was under construction while on ridge top of the roof approximately 30  foot from the lower level. Tar paper and Shingles were falling back down to the ground, employees were observed without hard hats exposing them to a struck by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about at 12/8/16 located at Southwick Dr., Cumming, GA: Employees were roofing an house that was under construction while installing tar paper and roofing shingles and while working at the edge, the employer did not ensure that fall protection was used by his employees while exposed to fall hazards of approximately up to 30- feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about at 12/8/16 located at Southwick Dr., Cumming, GA., Employees were observed and exposed to fall hazards, while roofing a new house, employees were using a portable extension ladder to gain access to the roof, and the ladder was only 18- inches above the landing surface.
