29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, were not protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires: a. 88 Flexo Machine - On or about December 7 and 8, 2016 employees were exposed to rotating horizontal unguarded shafts, in that guarding was not provided to employees operating the machinery. b. 66 X 130 Machine - On or about December 7 and 8, 2016 employees were exposed to rotating horizontal unguarded shafts, in that guarding was not provided to employees operating the machinery. c. Bobst Machine - On or about December 7 and 8, 2016 employees were exposed to rotating horizontal unguarded shafts, in that guarding was not provided to employees operating the machinery. d. 50 X 135 Machine - On or about December 7 and 8, 2016 employees were exposed to rotating horizontal unguarded shafts, in that guarding was not provided to employees operating the machinery.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, were not guarded in accordance with the standards specified in paragraphs (m) and (o) of this section: a. 88 Flexo Machine - On or about December 7 and 8, 2016, employees were exposed to in running nip point hazards, in that the belt and pulley guarding was not provided to employees operating the machinery. b.
