OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crush-by hazards: On or about 12/6/2016, in the warehouse, employees accessing the industrial racks were exposed to struck-by and crush hazards where the upright feet of the racking systems were visibly damaged by powered industrial trucks and were continued to be used at locations including but not limited to racks marked: a) A7-50-B-01/A7-48-B-01 b) A7-44-B2 c) A7-36_B-01 d) A7-27-B-03 e) A7-36-B-03 f) B3-55-A-01 g) B3-45-A-01 h) B3-39-A-01 i) B1-14-A-01 j) J4-18-B-02 k) J2-14-C-06 In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms: On or about 12/7/2016 in the L.U.M. area, employees were exposed to fall hazards of six inches to the powered conveyor, and 30 inches to the concrete floor below where the industrial stairs provided to employees to cross from one side of the 24 inch high conveyor to the other side were missing handrails.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: On or about 12/6/2016 throughout the warehouse, employees were exposed to amputation, electrical, caught-in, and struck-by hazards where the employer failed to enforce machine specific, group lockout energy control procedures and devices were not used when groups of two or more maintenance employees performed maintenance and servicing activities on equipment such as but not limited to the conveyor system which is electrical, pneumatic, gravity, and hydraulic and used only one lock. McKesson Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(f)(3)(i): which was contained in OSHA inspection number 1155146, citation number 1, item number 1 and was affirmed as a final order on (9/20/2016), with respect to a workplace located at 8313 Polk Lane Olive Branch, MS 38654.
