29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. WORKSHOP: The energy control procedures did not include the alternative measures to control hazardous energy sources for the employees during servicing and maintenance of equipment, such as (but not limited to) the repair line conveyor. On/or about December 1, 2016, an employee was performing service and maintenance on the Repair Line Conveyor which was still energized to replace a roller containing a defective sprocket, and the employee sustained a finger amputation injury due to a sudden start of the Repair Line Conveyor.
