29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4): (a) The employer is failing to protect employees when working on wood working machines in the production area of the plant. This was most recently documented on December, 8, 2016 at worksite 406 W 22nd St, Higginsville, Missouri.-During inspection it was discovered the employer had not developed a lockout tagout program for isolating potential energy of equipment and provided no training to authorized employees. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) The employer is failing to protect employees when performing maintenance and service on mill working machines in the production area of the plant. This was most recently documented on December, 8, 2016 at worksite 406 W 22nd St, Higginsville, Missouri.-Authorized employees were exposed to amputation and crushing hazards , in that the employer had not developed specific energy control procedures for the various types of machinery and equipment relating to mill working activities. Employer failed to set forth specific requirements for testing the machine to determine and verify the effectiveness of lockout devices and other energy control procedures, including but not limited to the Diehl Accumould 5 head Moulder Model# DL406-3 Serial#97M-6243-5659. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

Employees were exposed to serious injuries to their hands and fingers, in that the employer did not ensure that the DeWalt Deluxe Power Shop radial saw Motor # 80095-59 Serial # 65030454 ,was provided with an adjustable hood guard that fully enclosed the saw blade above the table and the material being cut. (b) The employer is failing to protect employees this was most recently documented on or about December 8, 2016, at worksite 406 West 22nd St, Higginsville, Missouri. Employees were exposed to serious injuries to their hands and fingers, in that the employer did not ensure that the DeWalt Power Shop Model 500268 Serial # 62120509,was provided with an adjustable hood guard that fully enclosed the saw blade above the table and the material being cut.
