29 CFR 1910.269(d)(2)(iv): The procedure did not clearly and specifically outline the scope, purpose, responsibility, authorization, rules, and techniques to be applied to the control of hazardous energy, and the measures to enforce compliance: Offutt AFB, on 7 December 2016, the employer (55th Wing) did not direct in the squadron level checklist attached to the Form 269 that the employees review a schematic diagram of the electrical connection between the A & B sides of the 13,800V switch in annex building 500 of the old USSTRATCOM facility prior to their engaging in the safe clearance and Lock Out Tag Out procedures, during which an arc flash occurred.

29 CFR 1910.269(l)(8)(ii): For each employee exposed to hazards from electric arcs, the employer did not make a reasonable estimate of the incident heat energy to which the employee would be exposed. The employer is failing to ensure that all employees exposed to electric arc hazards are aware of potential heat energy to which they may be exposed. This most recently occurred on December 7, 2016 on the jobsite located on Offutt Air Force Base. Civil Engineer employees were working on a 13,800V switch in annex building 500 of the old U.S. Strategic Command facility where there was an arc flash incident.
