OSH ACT of 1970 Section (5)(a)(1):   The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to amputation and crushing hazards:  On or about 11/22/2016, an employee was operating a 3M Truing and Dressing Machine when he failed to follow the manufacturer's operating instructions/warnings related to not putting hands near moving or rotating parts of the machine. An amputation of one finger and the crushing of three others resulted after a failed attempt at cleaning off the rotating work wheel which led to his hand getting pulled into the ingoing nip between the truing wheel and the work wheel.
