29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. On or about 12/08/2016, plastic pellets and trimmings that have spilled on to the work floors was not cleaned up in a regular basis. On 12/05/2016, near press number 7, the operator slipped and fell as the result of stepping on the plastic pellets and trimmings. The employee sustained broken femur.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 12/08/2016, a robot feeding supplies to press 7 that was kept in 6 foot fence was not interlocked to keep workers from accessing the area while the robot was running.
