29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) Employees engaged in rasping activities on the south scaffold were exposed to fall hazards of approximately 5 ft in that the lowest working level of the scaffold was not fully planked or decked.

29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section: Employees engaged in construction activities near the south scaffold were exposed to struck by hazards from falling objects approximately 58 ft above ground level in that entering the barricaded area was not prohibited, toeboards were not installed, and no other equivalent protection was provided.
