29 CFR 1926.850(c): All electric, gas, water, steam, sewer, and other service lines were not shut off, capped, or otherwise controlled, outside the building line before demolition work is started. In each case, any utility company which is involved shall be notified in advance: a) Jobsite, 155 Cochran Road, Pittsburgh, PA: On or about December 11, 2016, two employees where exposed to electrical hazards while performing demolition of a manhole that contained electrical wires that were still energized at 23kv and the electric company whose name was on the outside of the manhole cover, was not notified concerning that particular manhole.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: a) Jobsite, 155 Cochran Road, Pittsburgh, PA: On or about December 11, 2016, two employees were exposed to electrical hazards while trying to remove an energized 23kv electric line from a manhole and had not been trained how to recognize the danger of an electrical wire if it is not known if it is energized or not and how to avoid the unsafe condition which would have the potential to occur during the course of work the company primarily performs, excavating.
