29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a. Production and blending Areas - On or about December 7, 2016, employees were required to work with hazardous chemicals such as corrosive mixtures containing potassium hydroxide (KOH) and facilities for quick drenching or flushing of the eyes were not provided for immediate emergency use. Employees were thereby exposed to serious eye damages. 1. 2nd Floor - Oil Tankers/By Fire Extinguisher # 24 - the bowl type eye wash station showed no water stream/flow through the spray heads when the push-valve was activated. 2. 2nd Floor/Wall Mounted/By the Bradley Wall Signs - the bowl type eye wash station showed weak water stream/flow through the spray heads when the push-valve was activated. In addition, there was no collection tank for used water.

First Floor/Employee's Washroom Facilities - On or about December 7, 2016, the employees required to manufacturer chemical products were not provided with appropriate washing facilities. Employees were thereby exposed to unsanitary conditions. 1. At the first floor/Ladie's washroom - there was only cold - temperature - running water. 2. At the first floor/Men's washroom - there was no running water at all.
