29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a)Maintenance Area: On or about December 5, 2016 and times prior, an employee used a DeWalt 6" inch abrasive bench grinder (0229-YL0497) to clean up a bolt. The right side adjustable tongue guard on the grinder was not adjusted resulting in a 3/4 inch opening between the abrasive wheel and the tongue guard.
