29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) Workshop: On or about October 6, 2016 and times prior, an employee used a Dayton 8 inch abrasive bench grinder to knock spurs off of cut metal. The adjustable work rest was not adjusted on the left side resulting in a 1/2 inch opening between the wheel and the work rest.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a)Dayton 8 inch Bench Grinder Right Side: On or about October 6, 2016 and times prior, an employee used a Dayton 8 inch abrasive bench grinder to knock the spurs off cut metal. The grinder did not have an adjustable tongue guard installed resulting in 1/2 inch opening between the abrasive wheel and the top of the grinder. b)Dayton 8 inch Bench Grinder Left Side: On or about October 6, 2016 and times prior, an employee used a Dayton 8 inch abrasive bench grinder to knock the spurs off cut metal. The grinder did not have an adjustable tongue guard installed resulting in 1/2 inch opening between the abrasive wheel and the top of the grinder.
