29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section in situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof: a) Keystone Powdered Metal Company, on or about December 7, 2016, and times prior thereto, employees performing servicing and/ maintenance operations on machinery including, but not limited to, the Cincinnati Rapid-Reflex 825 Ton Powdered Metal Press, were not removed from machinery when the equipment needed to be energized to reposition components of the machine.
