On or about November 30, 2016, at the elevator machine room located in 25 W. Forsyth St., Jacksonville, FL, the employer did not ensure lockout/tagout procedures for the control of hazardous energy were being utilized while servicing elevator equipment with potential to store energy, exposing employees to amputation hazards.
