29 CFR 1928.57(a)(11): Where a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish the criteria in 1928.57(a)(11)(i-iii):     (a)  In the ginning area, on or about December 4, 2016, the employer did not ensure that each authorized employee (an employee who's duties include performing servicing or maintenance) received training in the methods and means necessary for energy isolation and control with regards to the lower system of the Stick Machine creating an amputation hazard.    (b)  In the ginning area, on or about December 4, 2016, the employer did not ensure that lockout or tagout devices were affixed to each energy isolating device by all authorized employees with regards to the lower system of the Stick Machine creating an amputation hazard.    (c)  In the ginning area, on or about December 4, 2016, the employer did not ensure that, prior to starting work on the lower system of the Stick Machine, an authorized employee had verified that isolation and deenergization of the machine had been accomplished creating an amputation hazard.    (d)  In the ginning area, on or about December 4, 2016, the employer created an amputation hazard for employees performing group maintenance on the lower system of the Stick Machine by:  failing to ascertain the exposure status of individual group members with regard to the lockout or tagout of the machine;  failing to ensure continuity of protection for each individual group member; and, failing to ensure each authorized employee affixed a personal lockout or tagout device to a group lockout device, group lockbox, or comparable mechanism at the beginning of work.
