29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):       a) Frank Donio Inc. - Throughout the production Area: The energy control procedures were not specific for each individual type of machines including but not limited to: Hydrolec Trash Compactor, Model #HE15264, S/N 080211RP0918, Roach Conveyors, Schurstar Bagger machine Model # 2060-C, Kallfass Film Packing machine Type Universa 500, machine #M01-12.914, etc.   The program only has a one fits all LO/TO procedure and it did not clearly and specifically identify the placement, removal or transfer of Lockout /Tagout devices on each individual type of machine and did not specify the requirements for testing to verify the effectiveness of LO/TO devices and other control measures. Occurred on or about 12/07/2016.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) Frank Donio Inc. - Production area:  The employer did not provide an eye wash station that provides 15 minutes of continue flow for flushing of the eyes.  Employees working in the production area can come in contact with corrosive chemicals such as Sparchlor Chlorinated Sanitizer and the chlorinated degreaser while performing machine sanitation operations. Occurred on or about 12/07/2016.
