29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about November 27, 2016, employees engaged in manufacturing activities, clearing a tape jam from the TACE 71 machine located in the sun chips department, were exposed to struck-by and caught-between hazards in that machine guarding was not provided to protect the employees from the moving components of the TACE 71 machine.

29 CFR 1910.178(a)(6): The employer did not ensure that all name-plates and markings were in place and maintained in a legible condition: Employee in the shipping area was exposed to struck-by hazards while operating the S3 CROWN. Model C-5 1000-50, Serial Number 9A229071, LP Gas Forklift. The forklift truck right control lever which moves the forks, did not have the control lever knob marked to identify the function and pattern movement of the controls.
