29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a)  On or about December 9, 2016, the employer did not ensure that employees were restrained within the aerial lift bucket to prevent ejection, while working at heights approximately 18 feet from ground to platform of aerial lift, which was located at 4045 Sinton, Colorado Springs, Co 80907.  Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
