29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a. On or about December 8, 2016, at the east side of the residential building, an employee using a pneumatic nail gun to install wall sheathing was not wearing eye protection, and exposed to flying debris and struck-by hazards. b. On or about December 8, 2016, at the west side of the residential building, an employee using a radial saw to cut wood while in the ground was not wearing eye protection, and exposed to flying debris and struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a. On or about December 8, 2016, at the west side of the building, two employees doing sheathing work on a 6:12 pitch residential roof were not protected from a 13ft-8in fall hazard by the use of a fall protection system. b. On or about December 8, 2016, at the west side of the building, one employee doing leading edge sheathing work on the roof ridge was not protected from a 25 feet fall hazard by the use of a fall protection system. c. On or about December 8, 2016, at the west side of the building, one employee doing framing work while standing and walking inside the ceiling beam was not protected from a 12ft-4in fall hazard by the use of a fall protection system.
