29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s). On December 5, 2016 and times prior thereto, at the worksite located at 555 McKee Drive in Gentry, Arkansas, the Line 308 Wrapper machine #4 (McKee Equipment Number 008047) was not fully guarded to keep employee's hands from entering the point of operation. This condition exposed employees to amputation hazards.
