On 12/5/2016, at the Peachtree Corners facility, at the base of the MRF Line, employees were exposed to tripping hazards in emergency egress paths as a pipe intended to weight down a canvas curtain was permitted to extend into the exit path at the base of the platform.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent.  On 12/5/2016, at the Peachtree Corners facility, the employer had not implemented a Hearing Conservation Program for employees when noise levels exceeded 85 db during an 8 hour exposure.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On 12/5/2016, at the Peachtree Corners facility, the employer did not implement a Lockout Program for employees providing service and maintenance and other employees working around those employees so as to control unexpected startup of those machines or equipment. On 11/28/2016, an employee incurred an amputation while removing a jammed piece of lumber from a conveyor chute without the protection of a Lockout Program.
