29 CFR 1910.119(e)(1): The process hazard analysis did not identify, evaluate, and address the control of the hazards involved in the process: On or about December 2, 2016, and at times prior thereto, the employer did not adequately identify, evaluate, and address the control of hazards associated with the chlorine production process. Specifically, the employer was not able to adequately operate chlorine scrubbers when the facility experienced a loss of power. As a result, employees were exposed to chlorine released from the facility.
