29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      (a)     On or about December 12, 2016, an employee installing windows while standing on the roof at a height of approximately 11 feet was not protected from falling to the lower level with guardrail, safety net or personal fall arrest systems.

29 CFR 1926.1053(b)(7): Ladders were used on slippery surfaces:    (a)     On or about December 12, 2016, two Werner 24-foot fiberglass extension ladders used to access the second floor sidewall windows were used on the snow-covered concrete patio.  The ladders were not lashed or secured from slipping.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:      (a)     On or about December 12, 2016, the employer did not record the following workplace injuries and illnesses on the OSHA Form 300 or equivalent for the calendar year 2015.    1.     On the 2015 OSHA Form 300 Log of Injuries and Illnesses there were no boxes checked in columns G, H, I or J for the eight injuries that occurred that year such as case number 1 through case number 8.      (b)     On or about December 12, 2016, the employer did not record the following workplace injuries and illnesses on the OSHA Form 300 or equivalent for the calendar year 2016.      1.     On the 2016 OSHA Form 300 Log of Injuries and Illnesses there were no boxes checked in columns G, H, I or J for the five injuries that occurred that year such as case number 1 through case number 5.
