29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:   a) Exterior of the Building - Front: Two employees engaged in framing activities were working from a wooden pallet as a platform supported by an All Terrain Forklift  being exposed to a fall hazard of approximately 10 feet, on or about 12/3/16.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:    a) Garage Entrance: Employees were exposed to tripping hazards as they entered the house climbing a 24" step, on or about 12/3/16.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:          a) Exterior of Building - Front Door: Employee was exposed to head injury while walking under the elevated platform without the use of protective helmet where two other employees were preparing to install a window, on or about 12/3/16.   G. M. P. Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1031392, citation number 01, item number 01 and was affirmed as a final order on 2/8/16, with respect to a workplace located at 43 Masonicus Rd., Mahwah, NJ.
