29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: (a) Warehouse - A dead bird and other bird debris, such as feathers and waste, had collected in the ceiling fan marked with the number 13, exposing employees to pathogenic hazards, on or about December 13, 2016. (b) Warehouse - Bird debris, such as feathers and waste, had collected in the ceiling fan marked with the number 1, exposing employees to pathogenic hazards, on or about December 13, 2016. (c) Warehouse - Bird waste had collected on top of a box located under the ceiling fan marked with the number 1, exposing employees to pathogenic hazards, on or about December 13, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1904.4(a): Each employer required by this part to keep records of fatalities, injuries, and illnesses did not record each fatality, injury and illness that was work-related, a new case, and that met one or more of the general recording criteria: (a) TST Impreso, Inc., 1002 Wayne Avenue, Chambersburg, Pennsylvania - On or about December 13, 2016, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2016: On or about August 11, 2016, an employee received stitches due to a laceration on the left hand from getting struck-by a broken grinding wheel. On or about August 29, 2016, an employee had days away from work and required prescription medications due to an eye infection that developed after the employee had bird droppings and feathers fall into his eye. On or about October 6, 2016, an employee received stitches due to a laceration of the right thumb from getting cut by a strap cutter. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
