29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:    The employer is failing to protect employees from fall hazards while performing steel erection activities.  This was most recently documented on December 10th, 2016 at the job site located at 401 W 4th Street, Kearney, Nebraska 68845 where employees were exposed to falls of greater than 30 feet without fall protection.  An employee fell 32 feet from the roof to the concrete floor slab inside the building while removing and installing standing seam metal roof panels.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:    The employer is failing to protect employees from fall hazards by failing to train each employee performing steel erection activities.  This was most recently documented on December 10th, 2016 at the job site located at 401 W 4th Street, Kearney, Nebraska 68845.  Employees performing steel erection activities had not been trained as to the nature of fall hazard associated with work and the appropriate fall protection equipment to use to protect themselves.
