On or about November 29, 2016, at 605 Satellite Blvd. NW Suite 100 Suwanee GA 30024 a)Production Area - Employees using end cap machines including but not limited to C2, were exposed to amputation hazards as an employee removing a jam from the machine did not utilize a lockout procedure when performing that task.
