29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  On or about December 13, 2016 and at times prior thereto, at the worksite located at or near 8408 Trail Creek Drive, Sherwood, Arkansas, an employee was working below other employees who were performing framing construction overhead without wearing a hard hat. This condition exposed the employee to struck-by injuries to the head.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about December 13, 2016 and at times prior thereto, at the worksite located at or near 8408 Trail Creek Drive, Sherwood, Arkansas, an employee was using a circular saw to cut boards for residential framing without eye protection. This hazard exposed the employee to struck by injuries to the eye.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about December 13, 2016 and at times prior thereto, at the worksite located at or near 8408 Trail Creek Drive, Sherwood, Arkansas, an employee was standing on a wooden frame while performing framing construction of a new residential home without fall protection, 10 feet above the ground below. This condition exposed the employee to the hazard of falling to the ground below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step.  On or about December 13, 2016 and at times prior thereto, at the worksite located at or near 8408 Trail Cerek Drive, Sherwood, Arkansas, employees used the top step of a six foot stepladder to access the frame of the residential home that was being constructed. This condition exposed employees to the hazard of falling to the concrete pad below.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  On or about December 13, 2016 and at times prior thereto, at the worksite located at or near 8408 Trail Creek Drive, Sherwood, Arkansas, an extension cord that was connected to a circular saw did not provide strain relief. This condition exposed employees to electrical hazards.
