29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a) Employees performed grinding of steel members in the main fabrication shop using electric angle grinders, where personal monitoring on 1/31/2017 measured exposure to noise at 191% and 80% Dose; equivalent to 8-hour Time-weighted average of 94.6 dBA and 88.4 dBA respectively.  The employer did not implement a hearing conservation program for employees with noise exposure exceeding the values in Table D-2, to include training & audiometric testing, both initially and at least annually thereafter.  Condition(s) were noted on or about 1/31/2017.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   a) Air-purifying respirators with replaceable filters are provided to fabrication shop welders for protection against metal fumes during welding.  A written respiratory protection program was not developed & implemented with worksite-specific provisions covering respirator selection, care and maintenance, as well as medical clearance and training of users. 29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service:  a) Within the fabrication shop and outside lot, employee(s) were exposed to injury when operating a Nissan forklift which was not inspected at least daily for deficiencies / conditions that would adversely affect the safety of the vehicle.  Both solid rubber rear tires exhibited numerous gouges and/or missing chunks around their perimeter that could adversely affect traveling under load; particularly on rough / uneven pavement outdoors.  Condition(s) were noted on or about 12/13/2016.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles located within 8 feet of the floor had live parts exposed to employee contact:  a) Dual receptacle outlet on the wall in the outside bay at the south end of the building was located less than 8 ft above the ground and was missing the fixture faceplate cover.  Employee(s) using the outlet receptacle as power source for their Hilti portable electric angle grinder(s) were exposed to electrical shock hazard, due to exposure to energized parts.
