29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) Employees performed grinding of steel members in the main fabrication shop using electric angle grinders, where personal monitoring on 1/31/2017 measured exposure to noise at 191% and 80% Dose; equivalent to 8-hour Time-weighted average of 94.6 dBA and 88.4 dBA respectively. The employer did not implement a hearing conservation program for employees with noise exposure exceeding the values in Table D-2, to include training & audiometric testing, both initially and at least annually thereafter. Condition(s) were noted on or about 1/31/2017.
