29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket while working from an aerial lift. On or about 12/14/2016, at the above addressed jobsite, employees were engaged in demolition activities at approximately 15 feet above ground level working from an aerial lift without means of fall protection, exposing the employees to a fall hazard.
