29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees, the employer did not report the incident to OSHA: Ryder Truck Facility, Loading Area - On or about December 8, 2016, an employee fell from a Tommy lift gate on a truck and was hospitalized, due to a severe concussion. The employer failed to report this incident to OSHA, until December 13, 2016.
