Employee was exposed to hazards, including but not limited to: not being protected from falling more than 10 feet to a lower level.

Location: 2nd floor Roof on the north side of the jobsite      On or about 12/13/16, employees used a portable extension ladder for access to the roof to frame the roof. The ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed. The ladder also was not secured at its top.
