29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  (a) New residential construction located on Lot #11, Front side: On or about December 14, 2016, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic finish nail guns at 120 PSI, in that safety glasses or other eye protection was not provided to employees installing cornice boards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  (a) New residential construction located on Lot #11, Work bench: Employees were exposed to electrical shock hazards while using Makita circular saws, in that the electrical cords provided had damaged strain relief which exposed employees to live internal circuits in wet conditions.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a) New residential construction located on Lot #11, Work bench area: On or about December 14, 2016, and at times prior, the employer exposed employees to electrical shock hazards while operating circular saws with damaged electrical cords, in that the cords were torn, frayed and exposing the internal insulated wires as the conditions were wet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) New residential construction located on Lot #11, Porch roof: On or about December 14, 2016, and at times prior, the employer exposed employees to fall hazards of 13-feet from the lower level, in that a fall protection system was not provided to employees engaged in siding activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) New residential construction located on Lot #11, Front side: On or about December 14, 2016, and at times prior, the employer exposed employees to fall hazards of approximately 13-feet from the roof, in that the ladder provided was not extended at least 3-feet past the landing or secured to the structure to allow for safe access on and off the roof.
