29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use  The employer is failing to protect employees from hazards associated with ammonia leaks.  This was most recently documented at the Kraft-Heinz facility located at 2504 Industrial Road in Kirksville, Missouri.  The employer is allowing ice and rust accumulation on process equipment preventing visual inspection and compromising the integrity and operation of the system.    A.  An evaluation for ice accumulation had not been performed in all locations for remediation.  Ice accumulations were discovered in the following locations:  Valve HTLT - 09 (liquid transfer pump) Valve LTLT - 09 (Hansen valve) Compressor 3, Valve C3-01  Engine room Old System West Room  Compressor 1, Valve C1-01 Engine room Old System West Room  Lo temp accumulator (LTL pump vent #1 and LTL pump #1 suction)  High temperature NH3 accumulator   B.  An evaluation for corrosion had not been performed in all locations for remediation.  Corrosion issues were discovered in the following locations:  Valve HTLT - 13  Pump HTL transfer system HTLT-03 Pipe on top of the HTL system that goes to the accumulator Evaporator condenser EC9-11, Hansen 100 D Condenser 2 EC2-05

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):   Employees were exposed to an electrical shock hazard.  Employees working on or in the area of the electrical control box for compressor #1 in the legacy engine room were exposed to live conductors.   The 110 volt control box was not covered to prevent contact with live conductors.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:(a)(LOCATION)(IDENTIFY EQUIPMENT AND CONDITION(S))(STATE HAZARD(S) TO EMPLOYEE(S)NOTE: KNOTS IN CORDS, WINDING WITH TAPE, SPECIAL FITTINGS OR OTHER APPROVED MEANS OF CONNECTION ARE ACCEPTABLE (NEC 40010) 1910  Employees were exposed to an electrical shock hazard.  Employees working under the Legacy chillers area were exposed to live conductors from an electrical extension that feed the sump pump.   The 110 volt extension cord was broken at the fitting and attached with electrical tape that do not prevent contact with live conductors.
