29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: a) Building Exterior: The employer did not provided training to employees who face a risk of electric shock when they are required to work in close proximity to energized electrical lines. On 12/12/2016 an employee received electrical shock and burns while moving an extension ladder within approximately 5 feet of a 4KV primary energized power line.

29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started: a) Building Exterior: Employees were exposed to dangers such as electrical shock and electrocution hazard while working from a 40 foot extension ladder located approximately 5 feet from a 4KV energized overhead electrical line, on or about 12/12/2016.
