29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:      a) Throughout facility - On or about 12/15/2016 and at times prior, employees were exposed to upper respiratory tract, skin and eye irritations, in that a respiratory protection program was not implemented for employees required to wear 3M half mask respirators while welding metal parts made of bronze and copper and processing silica sands.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Copper Fume listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 0.1 mg/m3 TWA:    a) Production Shop - On or about 01/10/2017, an employee was exposed to Copper Fumes at an 8-hour TWA of 0.67 mg/m3 of air while welding metal parts. Sampling was performed for 360 minutes and zero exposure was assumed for that 120 minute period not sampled.     b) Production Shop - On or about 01/10/2017, an employee was exposed to Copper Fumes at an 8-hour TWA of 0.62 mg/m3 of air while welding metal parts. Sampling was performed for 405 minutes and zero exposure was assumed for that 75 minute period not sampled.     c) Production Shop - On or about 01/10/2017, an employee was exposed to Copper Fumes at an 8-hour TWA of 0.41 mg/m3 of air while welding metal parts. Sampling was performed for 290 minutes and zero exposure was assumed for that 190 minute period not sampled.
