29 CFR 1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive initial training in other unique or potentially hazardous conditions of the workplace that could affect safe operation:  Throughout Yard/Facility - October 4, 2016, the employer did ensure that powered industrial truck operators were sufficiently trained to recognize and understand the hazards associated with utilizing powered industrial trucks to lift or raise up other powered industrial trucks that had become stuck in gravel, mud, or other outdoors terrain, thus exposing employees to potential crushed-by or struck-by hazards.
