29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) The Gordon Lumber Company dba Gordon Components on or about December 15, 2016, at the address of 6485 CR 236 Findlay, OH: The employer did not ensure an energy control program consisting of machine specific control procedures, employee training and periodic inspections were developed and performed. Employees performing sawyer duties to include daily cleanout at or near the point of operation on the Mitek Cybersaws #1 and #2 and the TCT Saw without the benefit of energy isolation, hazardous energy recognition training and employer evaluation, were exposed to laceration(s)/amputation(s) in the event of unexpected saw startup.
