29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) Southwest corner of the main floor, work area behind the Produce Cases: Two (2) floor holes located in the work area had covers made of scrap lumber which were placed over each hole, but were not secured from movement.
