29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees:  a) Southwest corner of the main floor, work area behind the Produce Cases: Employees were working near two (2) floor holes located in the work area had covers made of scrap lumber which were placed over each hole, but were not secured from movement.
