29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. Site - Employees using a fiberglass extension ladder to access the roof were not protected from falls of approximately 12 feet when the side rails were only extended approximately 1 foot above the roofs eave. b. Site - Employees using an aluminum multi-position ladder, positioned as an extension ladder, to access the roof were not protected from falls of approximately 12 feet when the side rails did not extend above the roofs eave.
