29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   Northside of building: On or about December 14, 2016, employees were exposed to a fall hazard, while using a Skyjack SJIII Scissor Lift, Model 3220, to install down spouts on a commercial building without its midrail access chain attached.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system:  North side:  On or about December 14, 2016, employees without fall protection were exposed to fall hazards greater than 6 feet to the paved ground below, while installing down spouts on the roof of a commercial building.

29 CFR 1926.1053(b)(1): When a portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  North side: On or about December 14, 2016, employees were exposed to fall hazards, when using a ladder that was less than 3 feet above the upper landing surface, while gaining access to the pitched section of the roof.
