29 CFR 1926.306(b)(3)(iii): Safety appliances, such as valves, indicating devices and controlling devices were not constructed, located, and installed so that they could not be readily rendered inoperative by any means, including elements: Site - On 12/14/2017 an employee was exposed to struck by hazards while working inside a catch basin when a Cherne Industries Muni-Ball #355248, 12"-24" Pneumatic Plug was over pressurized when the employee was in the process of conducting a pressure test on a sewer line with equipment that did not have a properly functioning indicating device.
